If this is a first-time order or you require an additional refill(s) before your next scheduled

service call, then please fill out this order form and we will respond as quickly as possible. It is very important that you answer all the questions and hit the submit button when finished. We will correspond via email or telephone concerning your scheduled delivery date and any additional information once we have received your request. You may order one or both products using this same order form.

Please read the listed directions below carefully before placing your order

Spot Out® (stain remover) – Your Spot Out refill will automatically occur on your next service call at no charge to you. Should you require a refill before hand, we ask that you return your old bottle helping us keep our costs down allowing this program to continue. On the morning of your scheduled refill delivery, please set your empty Spot Out container outside your front door for pick up. We will deliver your new bottle of Spot Out inside a clear plastic bag and hang it on your inner most *exterior front door. If your original bottle of Spot Out has been discarded, there will be a $5.00 container replacement charge invoiced to you at the time of delivery. There will be no charge for future refills to your replacement bottle of Spot Out during of your next service call.

Bottom Line® (pet odor remover) – If you're requesting Bottom Line for the first time please fill out and submit the order form below. If you are in need of a refill, it will automatically occur on your next service call at no charge to you. Should you require a refill before hand, we ask that you return your old bottle helping us keep our costs down allowing this program to continue. On the morning of your scheduled refill delivery, please set your empty Bottom Line container outside your front door for pick up. We will deliver your new bottle of Bottom Line inside a clear plastic bag and hang it on your inner most *exterior front door. There will be a product charge of $15.00 invoiced to you at the time of delivery. If your original bottle of Bottom Line has been discarded, there will be an additional $5.00 replacement container charge bringing the total to $20.00. There will be no charge for future refills to your replacement bottle of Bottom Line during your next service call.

*During the months of November thru March, please remember to leave your exterior storm door unlocked. Placing the product between your exterior doors will help ensure that it does not freeze or experience any damage from the colder temperatures that could adversely affect results and/or product performance. If you do not have an exterior storm door available, then please provide an insulated container with a lid on your front porch as a substitute.
